Does my child qualify?
Most Florida children qualify for one of the four Florida KidCare programs. Learn more about the requirements below.
Requirements
Florida KidCare is for children, not adults. To qualify for low-cost coverage, a child must live in Florida, be under age 19, not reside in a public institution, and:
- Have a Social Security number (SSN) or have applied for an SSN
- Meet income eligibility requirements
- Be a U.S. citizen, an eligible immigrant or a lawful permanent resident (LPR)
Income Requirements
As an income-based program, monthly payment costs are based on a child’s age, household size and adjusted gross annual income (this is your total income minus deductions or adjustments). Federal Poverty Level Guidelines, set by the federal government, are used to determine families' monthly costs.
Citizenship and Identity Requirements
All new Florida KidCare enrollees must provide proof of U.S. citizenship and immigration status or lawful permanent residence and identity. Renewing Florida KidCare enrollees may have to provide new immigration documents if those documents expired.
To make things easier for our families, Florida KidCare checks this information with our agency partners. As a result, you may not need to provide any additional information at all.
If Florida KidCare is unable to find all needed information, you will be asked to provide documents. Examples of acceptable documentation are listed below.

Acceptable Forms of Documentation for U.S. Citizenship, Immigration or Lawful Permanent Resident and Identity
List A
Send a copy of only one of these documents.
U.S. citizenship and identity
- A U.S. Passport
- Expired U.S. Passport issued without limitation
- Certificate of Naturalization issued by U.S. Citizenship and Immigration Services (USCIS) (Form N-550 or N-570)
- Certificate of U.S. Citizenship issued by USCIS (Form N-560, N-560-A, N-560AB or N-561)
- Valid U.S. state-issued driver’s license if the state required proof of U.S. citizenship or verified the Social Security number before issuing the license
- Document issued by a federally recognized Indian Tribe, including tribes located in a state that has an international border, that:
Immigrant or LPR and identity
- Form I-94 or passport indicating admission to U.S. with nonimmigrant visa
- I-766 EAD
- I-551
List B
Send a copy of one of these documents plus one or two documents from List C as instructed.
U.S. Citizenship and Identity
- A U.S. public birth record (birth certificate)
- Certification of report of birth issued by the Department of State (Forms FS-240 or DS-1350)
- Final adoption decree showing the child was born in a U.S. state or territory
- U.S. military record showing the child was born in a U.S. state or territory
- Documentation that a child meets the requirements of section 101 of the Child Citizenship Act of 2000 as amended (8 U.S.C. 1431)
- Medical records, including, but not limited to, hospital, clinic, or doctor records or admission papers from a nursing facility, skilled care facility, or other institution that indicate the child was born in a U.S. state or territory
- Life, health, or other insurance record showing the date of birth and that the child was born in a U.S. state or territory
- Official religious record recorded in the U.S. showing the date of birth and that the child was born in a U.S. state or territory
- School records, including preschool, Head Start and daycare, showing the child’s name, date of birth and that the child was born in a U.S. state or territory
- Certified federal or state census record showing U.S. citizenship or that the child was born in a U.S. state or territory
- Affidavit signed by another person under penalty of perjury who can reasonably attest to the child’s citizenship and that contains the child’s name, date of birth and that the child was born in a U.S. state or territory
Immigrant or LPR
- I-797 approval notice
- Proof of I-918 submission
- Receipt from form I-102 or I-918
List C
Send a copy of one of these documents plus one document from List B.
Identity
- Documents for List C must contain consistent information, confirm the child’s identity and have a photograph or other identifying information that establishes the child’s identity, including, but not limited to, name, age, sex, race, height, weight, eye color, or home address.
- Identification card issued by U.S. federal, state, or local government agencies
- Driver’s license issued by a U.S. state or territory
- School identification card
- Military dependent’s identification card
- Clinic, doctor, hospital, or school record, including preschool or daycare records
- Identity Affidavit Form (only for children under age 16)
Or
Send copies of two of these plus one from List B.
For children 16 years of age or older:
- U.S. voter registration card
- U.S. military card or draft record
- U.S. Coast Guard Merchant Mariner Card
- Employer identification card
- High school diploma or high school equivalency diploma (GED)
- College diploma
